| Failure of 2400 lbs thrust rocket motor in flight at Balls 2005 (2011-06-17) Flight test of a 6 inch diameter 2400 lbs thrust solid rocket motor. Rocket weight at takeoff = 145 lbs. Motor was designed for a two stage 220 lbs rocket, but rocket was not ready in time and a lighter weight single stage airframe was substituted. As a result, the thrust to weight ratio was 16 to 1 (too high) instead of a more sane 10 to 1. Recovered flight data indicated acceleration exceeded 40 g's. Propellant grains collapsed under the high acceleration causing the chamber pressure to exceed the max design pressure of the motor case. The forward closure failed, ejecting the forward bulkhead and separating the airframe. The rocket tumbled end over end at ~Mach 2, and the resulting aerodynamic loads shredded it into pieces which fall and impact the desert. Balls Experimental Rocket Launch Black Rock Desert Nevada Sept. 19, 2005
|
| Failure of 4.5 inch solid rocket motor in flight (2011-06-17) 4.5 inch (114mm) diameter solid rocket motor with a D-slot grain fails in flight. The grain was recovered and the cause of the failure was over pressurization of the motor case due to the D-slot grain not being completely inhibited by the spiral wound phenolic casting tube. The spiral grooves in the casting tube allowed the flame front to burn on the backside of the grain, increasing the burning surface area (and chamber pressure) beyond the design limits of the motor. Rocket was a full scale model of an ARCAS HV. Balls Experimental Rocket Launch Black Rock Desert Nevada Sept. 19, 2005

| The Best Rocket Launch Ever 2006 (2011-06-28) A small private launch held at the Black Rock Desert Summer 2006. 6 inch OD ARLISS Rockets successfully flown on experimental 114mm N and 98mm M motors. A 3 inch minimum diameter rocket flown twice in same day to Mach 2 and 24000 ft on 7000 Nsec M motors. Really old motors from manufacturers long gone out of business. Perfect flying weather, great friends, successful flights on experimental rocket motors, dinner and drinks at Bruno's = the best rocket launch ever.

| Two M Motor Failures at LDRS XX (2011-06-22) A solid fuel M Motor flies out of control randomly around the launch pad while spectators run and watch. It is immediately followed by a hybrid "M" motor rocket failure. The hybrid motor is a blowdown type which causes a "buzzing" vibration that spoofs the avionics into triggering the ejection charge and deploying the parachute early while under boost.
